پروتکل‌های مداخله‌ای برای داستان‌گویی و درک استدلال شفاهی در کودکان دارای اختلال تکاملی زبان: یک گستره‌پژوهی

چکیده
مقدمه: اختلال تکاملی زبان (Developmental language disorder یا DLD) با مشکلات پایدار در بیان و یا درک زبان همراه است و داستان‌گویی و درک استدلال شفاهی از مهارت‌های پیچیده‌ی زبانی هستند که این کودکان اغلب در آن‌ها دچار اختلال‌ می‌باشند. پژوهش حاضر با هدف شناسایی پروتکل‌ها و برنامه‌های مداخله‌ای مورد استفاده برای بهبود داستان‌گویی شفاهی و درک استدلال شفاهی در کودکان دارای DLD انجام شد.
مواد و روش‌ها: این مطالعه از نوع گستره‌پژوهی بود. جستجوی منابع در بانک‌های اطلاعاتی داخلی و خارجی، با رعایت معیارهای ورود و خروج، طی بازه‌ زمانی سال ۲۰۰۳ تا دسامبر ۲۰۲۲ انجام شد. در نهایت، ۱۱ مقاله بررسی و نتایج در جدولی اختصاصی دسته‌بندی گردید.
یافته‌ها: بیشتر تحقیقات بر داستان‌گویی و سازماندهی روایت تمرکز داشتند. آموزش صریح ساختار داستان، بازگویی و تولید داستان، استفاده از مدل‌سازی و حمایت‌های دیداری- کلامی و هدف‌گیری هم‌زمان ساختار کلان و خرد روایت، از مؤلفه‌های پرتکرار بودند و بهبودهایی در گرامر داستان، سازماندهی روایت، پیچیدگی زبانی، انسجام و واژگان گزارش شد. پژوهش‌ها کمتر بر درک استدلال شفاهی متمرکز بودند و از راهبردهایی مانند خواندن تعاملی، پرسشگری هدایت ‌شده، تصویرسازی ذهنی و تمرین مستقیم استدلال استفاده کردند. قوی‌ترین شواهد مربوط به یک کارآزمایی بالینی تصادفی بود که بهبود درک استنباطی و حفظ و تعمیم اثر مداخله را نشان داد.
نتیجه‎گیری: شواهد موجود از ظرفیت بالقوه‌ مداخلات داستان‌محور و آموزش مستقیم استدلال شفاهی در کودکان دارای DLD حمایت می‌کند، اما ناهمگونی مداخلات و محدودیت مطالعات با طراحی تجربی قوی، امکان معرفی یک پروتکل استاندارد را محدود می‌سازد. به نظر می‌رسد رویکردی یکپارچه، صریح و ساختار یافته که آموزش روایت، حمایت دیداری- کلامی و اهداف واژگانی، نحوی و استدلالی را متناسب با سطح رشدی کودک ترکیب کند، مناسب‌ترین جهت برای مداخلات آینده باشد. فقدان مقالات فارسی، ضرورت انجام پژوهش‌های بومی را برجسته می‌کند.
