تأثیر تمرینات ثبات عصبی‌- عضلانی پویا همراه با بازخورد دیداری آینه در مقایسه با تمرین بدون بازخورد آینه بر عملکرد تنفسی بیماران مبتلا به بیماری پارکینسون: تحلیل ثانویه از یک کارآزمایی کنترل‌ شده تصادفی

چکیده
مقدمه: بیماری پارکینسون می‌تواند قدرت عضلات تنفسی را مختل کند و در نتیجه، موجب کاهش کارایی ریه و کاهش تحمل فعالیت شود. تمرینات تثبیت
عصبی‌- ‌عضلانی پویا (
Dynamic neuromuscular stabilization یا DNS) موجب بهبود کنترل تنه و مرکز بدن می‌شود. همچنین، بازخورد دیداری آینه (Mirror visual feedback یا MVF) می‌تواند یکپارچگی حسی‌- حرکتی را بهبود بخشد. با این حال، اثر آن بر پیامدهای تنفسی در بیماران مبتلا به پارکینسون هنوز مشخص نیست.
مواد و روش‌ها: پژوهش حاضر، آنالیز ثانویه یافته‌های افراد مبتلا به پارکینسون در یک مطالعه کارآزمایی بالینی می‌باشد که یک برنامه تمرینی هشت ‌هفته‌ای DNS را تکمیل کردند. این برنامه شامل سه جلسه در هفته و هر ‌جلسه ۴۵ تا ۶۰ دقیقه بود. تمرینات یا در برابر آینه تمام ‌قد یا بدون آینه انجام گردید. ارزیابی‌های اسپیرومتری شامل شاخص‌های ظرفیت حیاتی اجباری، حجم بازدمی اجباری در ثانیه اول (FEV₁)، نسبت Forced vital capacity (FVC)FEV₁/، جریان بازدمی اجباری در 75-25 درصد ظرفیت حیاتی (FEF25-75) و اوج جریان بازدمی مورد بررسی قرار گرفت. داده‌های ۳۴ شرکت‌کننده شامل ۱۷ نفر در هر گروه با استفاده از آزمون Two-way repeated measures ANOVA و آزمون اثرات بین‌آزمودنی تحلیل گردید.
یافته‌ها: هر دو گروه در طول زمان در همه شاخص‌های اسپیرومتری بهبود معنی‌دار درون‌گروهی را گزارش کردند (001/0 > P). با این حال، گروه DNS + MVF در مقایسه با گروه DNS-only، بهبودی معنی‌دار بیشتری را در FVC، FEV₁، FEF25-75 و Peak expiratory flow (PEF) نشان داد. برای FVC، تعامل گروه × زمان معنی‌دار بود (03/72 = F، 001/0 > P، η² جزئی = 873/0). برای FEV₁ نیز تعامل گروه × زمان معنی‌دار بود. همچنین، برای FEF25-75 و PEF مقدار گزارش شد. اثرات بین‌آزمودنی نیز برای FVC و PEF به نفع گروه آینه بود (به ترتیب 008/0 = P و 043/0 = P).
نتیجه‎گیری: نتایج نشان داد که DNS + MVF می‌تواند به منظور بهبود عملکرد تنفسی در بیماران مبتلا به پارکینسون استفاده شود. انجام تحقیقات بیشتر با حجم نمونه بزرگ‌تر و پیگیری طولانی‌تر ضروری است.
