Identification of the beneficial factors involved in improving motor performance of these patients is clinically important. The aim of this study was to investigate the effects of a cognitive dual task on kinematic variables of the gait in patients with parkinson's disease. Materials and Methods: Ten female patients with parkinson's disease (Already on Levedopa treatment) with mean age, height and weight of 59.6 yrs ± 3.6 yrs , 157.2 cm ± 4.4 cm and 67.2 kg ± 6.7 kg were included in the experimental group. 14 healthy females with similar age, height and weight were served as the control group. Using a H-Rez 3D Motion Analysis system, the movements of lower limbs were captured and the related kinematic variables were analyzed during walking with and without cognitive dual task. MANOVAs was used for statistical analysis (µ=0.05). Results: Stride length of the control and experimental groups for normal walking were 121.9 cm ± 4.7 cm and 79.7 cm ± 6.1 cm , respectively (p=0.001). These values for walking under cognitive dual task for control and experimental groups were 127.3cm±  5.0cm and 86.4 cm ± 6.5 cm, respectively. In PD patients, stride length, walking speed, joints' range of motion and angular velocity at joints were of smaller amounts than those of healthy subjects. However, the double support in patients were greater than the normal subjects (p=0.001).  Cognitive dual task resulted in an increase in the stride length, walking speed, hip's range of motion and a decrease in the double support time.Conclusion, Cognitive dual task can improve the walking pattern and is recommended to be implemented in rehabilitation programs designed for PD patients.Keywords: Parkinson's disease, Gait, Cognitive dual task, stride length, stance, swing}, keywords = {}, title_fa = {تأثیر اضافه نمودن یک وظیفه ثانویه شناختی بر متغیرهای کینماتیکی راه رفتن در بیماران مبتلا به پارکینسون}, abstract_fa = {چکیدهمقدمه: بیماری پارکینسون با ضعف کنترل وضعیت بدن (Posture) و افت عملکرد حرکتی، به ویژه در راه رفتن، همراه است. شناسایی عواملی که در بهبود عملکرد حرکتی این بیماران مؤثرند از اهمیت کلینیکی برخوردار است. هدف از انجام این تحقیق، ارزیابی تأثیر یک وظیفه شناختی ثانوی بر متغیرهای کینماتیک راه رفتن در بیماران مبتلا به پارکینسون بود.مواد و روش‌ها: تعداد 10 بیمار زن مبتلا به پارکینسون تحت درمان با لوودوپا به ترتیب با میانگین سن، قد و وزن 6/3 ± 6/59 سال، 4/4 ± 2/157 سانتی‌متر و 7/6 ± 2/67 کیلوگرم به عنوان گروه تجربی و 14 زن سالم غیر ورزشکار با سن، قد و وزن مشابه به عنوان گروه شاهد در این تحقیق شرکت کردند. الگوی کینماتیک راه رفتن آزمودنی‌ها در حالت عادی و همراه با یک وظیفه شناختی ثانوی با استفاده از دستگاه تجزیه و تحلیل حرکتی (High-Rez Motion Analysis System) مورد اندازه گیری قرار گرفت. طرح آزمایش شامل یک عامل بین گروهی و دو عامل درون گروهی (هر یک با دو سطح) بود و برای تحلیل آماری از آزمون Repeated Measure (05/0 = µ) استفاده گردید.یافته‌ها: طول گام (Stride) افراد سالم و بیمار در راه رفتن عادی به ترتیب 7/4 ± 9/121 و 1/6 ± 7/79 و در راه رفتن همراه با اجرای وظیفه ثانوی شناختی 0/5 ± 3/127 و 5/6 ± 4/86 بود. در بیماران مبتلا به پارکینسون، طول گام، سرعت گام برداری، سرعت زاویه‌ای مفاصل اندام تحتانی، و دامنه حرکتی مفاصل اندام تحتانی کوچک‌تر از افراد سالم بود. اما، زمان حمایت دوگانه در بیماران به طور معنی‌داری بیشتر از این مقدار در افراد سالم بود (001/0 = P). میزان طول گام بیماران و دامنه حرکتی مفصل ران در راه رفتن با وظیفه ثانوی شناختی به طور معنی‌داری بهبود یافت (001/0 = P).بحث: وظیفه ثانوی شناختی هنگام راه رفتن موجب بهبود متغیرهای کینماتیکی راه رفتن می‌شود. افزودن وظیفه ثانوی شناختی هنگام تمرین راه رفتن در برنامه توان‌بخشی بیماران مبتلا به پارکینسون توصیه می‌گردد.کلید واژه‌ها: بیماری پارکینسون، راه رفتن، وظیفه ثانوی شناختی، طول گام، ایستایی، نوسان. }, keywords_fa = {}, url = {https://jrrs.mui.ac.ir/article_16384.html}, eprint = {https://jrrs.mui.ac.ir/article_16384_70b06386604409b52bc0a65af6aacd3e.pdf} }
